Required Documents

 

Please see our "Overview" page for information on the overall rental and reservation process. The following documents are required to secure your reservation:

  1. A copy of the signed contract

  2. A copy of the organization's 501(c)3 certification letter to waive rental fee    (Please note the organization name on the 501(c)3 letter must match the organization name on the request forms and contract. Fiscally sponsored organizations are subject to the $500 rental fee. The Foundation will only waive the rental fee for groups with their own 501(c)3 status).

  3. A check for all applicable fees   (Checks are made out to Cavalier Maintenance Services, Inc., but mailed to the Foundation's address at 1200 U Street). For more information on fees, please click here.

  4. A Certificate of Insurance (COI)

Certificate of Insurance:

The Certificate of Insurance must meet the Foundation's requirements. For instructions on how to properly complete a Certificate of Insurance for an event at the True Reformer Building, please click here. 

Government agencies must submit a "Statement of Self Insurance," which can be acquired through the DC Office of Risk Management. This "Statement of Self Insurance" serves as a rental fee waiver and as a Certificate of Insurance.
